TOOTH WEAR UPDATE.

10-11-12 November.

FRIDAY, 10 November 2023

Direct composites: anterior teeth monochromatic and layering techniques, posterior teeth – occlusal techniques, dealing with subgingival margins and difficult contact areas. Using the destination approach, how to refurbish and convert to stronger materials.

Theory:

Short seminar on how to design the smile with direct composite.

Practical:

Morning Hands-on anterior composite veneer to improve aesthetics, make the smile appear younger, while also managing early tooth wear.  Includes meeting the patient’s demands for life-like teeth or fashionable “perfect” teeth.

Afternoon – Hands-on learning how to maintain composites, refurbish them with new materials including ceramics.  Two posterior composites will be placed by each delegate learning how to bulk-fill difficult cavities, deal with subgingival margins and create good contact areas. Three techniques for how to quickly get the occlusion correct will be carried out by each delegate.

Objectives and learning outcomes. At the end of the day delegates will be able to:

  1. Assess a smile and design changes.
  2. Understand the options to show patients the design options beforehand.
  3. Place and finish a composite veneer with different levels of anatomy and detail to meet patient expectations.
  4. Maintain and upgrade composite veneers.
  5. Place a quick bulk-fill restoration with the correct occlusion without adjustments.
  6. Deal with subgingival margins.
  7. Create a good contact area.
  8. Apply three methods to get the occlusion correct.

SATURDAY, 11 November 2023

Indirect aesthetics: “preparation to cementation”. Teaching: onlays, overlays, crowns (range of materials), ceramic veneers (minimal, traditional, slice, 360)

Theory:

Seminar on managing damaged posterior teeth with suitable indirect restorations.

Practical:

Includes carrying out a pre-op aesthetic and functional (occlusal) assessment, selecting a suitable analgesic technique, indexing, all preparations for metal, ceramic and zirconia restorations including overlays, onlays, veneers, vonlays, partial and full crowns, including different margin designs and vertipreps. Impression recording, temporisation and cementation is covered.

Objectives and learning outcomes. At the end of the day delegates will be able to:

  1. Assess damaged teeth and select treatment options
  2. Use a range of preparation designs for different indirect materials to restore damaged teeth aesthetically and functionally.
  3. Recognize best options for aesthetics, damaged and root-filled teeth and bruxists.
  4. Use a range of impression techniques for difficult clinical situations eg subgingival margins, patients with excess saliva or large tongues.
  5. Use effective temporisation methods
  6. Know the best ways to cement restorations.
  7. Correctly check the occlusion after cementation.

SUNDAY, 12 November 2023

Managing tooth wear (direct and indirect techniques), correcting the OVD, correcting the occlusion, protecting teeth, correcting aesthetics. Includes splint therapies for TMD, clenching, bruxism and sleep apneas.

Theory:

This popular all-day course covers the aetiology and management of all types of tooth wear, from diagnosis and prevention through monitoring and management.

Practical:

Techniques for intervention use the latest range of adhesive aesthetic materials as well as traditional methods.  Hands-on sessions include tooth build-ups with a range of direct and indirect techniques.  Teaching covers the management of sensitivity, aesthetics and function as well as occlusal change in vertical and horizontal dimensions. Splint therapy is discussed and some splints fabricated.

Objectives and learning outcomes. At the end of the day delegates will be able to

  1. Assess patient for tooth wear, it is normal or excessive.
  2. Design a prevention strategy.
  3. Restore a tooth wear case, both localized and generalized.
  4. Understand when and how to change the occlusion.
  5. Know how and when to use Dahl and when not to use it.
  6. Use splints are appropriate in the management of tooth wear, particularly bruxists.
